As a son of a country doctor Hegar dedicated itself at the beginning of the pregnancy diagnostics, which was at that time still in the child shoes. 1864 he became successor of Otto mirror mountain at the chair for and birth assistance in Freiburg. In the year 1879 it introduces the Hegarstifte for the Dilatation of the Zervikalkanales. Later am occupied it with Genitaltuberkulose and the role of the Ovars in the female body.

With its writing "“the sex impulse. A social medicinische study"”, which appeared 1894, belongs to Hegar also to the first advocates of a negative Eugenik or eugenics. It demands eugenische measures, in order to make "“at least for the largest damage of the today's condition an end to limit and the emergence of frail miserable humans"”. 1905 it became honour member in this year of the society for eugenics, created in Berlin.
