Definition

The hypertrophische Kardiomyopathie is an illness of the heart muscle, with which it comes to the Verdickung (Hypertrophie) of the muscle, particularly in the range of the left chamber and the septum. One differentiates between an obstructive (restricting) and an non-obstructive form.

see also: Kardiomyopathie

A cause

The obstructive form is usually caused by a mutation on the on Chromosom 14. By the Hypertrophie it comes to the shifting of the discharge course of the left Herzkammer and thus to a functional Aortenstenose with increased pressure load of the left chamber. The moreover one a relaxation disturbance of the heart muscle is present, whereby to the Diastole fewer bloods in the Herzkammer can be taken up.

Symptoms

  1. Breath complaints (dyspnea)
  2. Angina Pectoris
  3. Swindle, Synkopen
  4. Heartbeat disturbances

Diagnostics

In the ELECTROCARDIOGRAM one finds, as with the dilatativen Kardiomyopathie Linkshypertrophiezeichen. The moreover one finds a linksanterioren Hemiblock and Septumhypertrophiezeichen. In the Echokardiographie sees one a Septumhypertrophie with restricting of the left chamber.

Therapy

The not-obstructive form does not require treatment. The obstructive form is treated as follows:

  1. physical indulgence
  2. Beta blocker
  3. Calcium antagonist
  4. Antiarrhythmics with heavy rhythm disturbances

Nitrate and substances such as Digitalis and Katecholaminee are contraindicated, since they can aggravate narrowing of the Ausflusstraktes.

The illness is concerned by surgery either by a muscle distance or a Herztransplantation. Alternatively since 1994 of the thickened muscles in the context of a heart catheter investigation by means of alcohol injection one Will small quantities of absolute alcohol with the help of a balloon catheter, as it for the Koronarangioplastie used are brought, into the appropriate Versorgungsarterie, which leads to a controlled cardiac infarct and to an extension of the discharge course. The most frequent side effect of this in the meantime widespread method is a disturbance of the attraction line of the heart; in approximately 10% of the cases therefore a cardiac pacemaker must be implantiert.
